How to use this form
- 1Pick the version that matches the borrowerAn individual borrowing in their own name, a registered business, or a lending group. Each asks only for what is relevant to that borrower, so nothing on the page goes to waste.

What the form asks for
- The loan applied for β purpose, amount, duration and preferred repayment cycle
- Identity: full name, date of birth, ID or passport number, and a photograph
- Marital status, gender and next of kin
- Employment, employer, and net monthly income
- For a business: registration number, legal form, owners and directors, turnover
- For a group: office bearers, a members roster and joint-and-several liability
- Contact details and residential or trading address
- Collateral offered, with its condition and estimated value
- A tick list of documents attached, a signed declaration, and an office-use panel

- Enough to identify the applicant and assess whether they can repay β identity, contact details, address, income and existing obligations. Beyond that, data protection law in most countries expects you to collect only what you will actually use in the decision. If a field on this form is not something you will look at, delete it before you print: an unused field is a liability, not a formality.
- The form has a tick list of the usual ones β a copy of the applicant's ID, proof of residence, payslips or business records, and a recent bank statement. What you actually require is your call and your regulator's, so treat the list as a starting point. Lenders operating in more than one country generally find the list changes market by market.
- No. An application is an offer to borrow, not a contract. Nothing binds either side until you approve it and both parties sign a loan agreement setting out the amount, rate, term and repayment obligations. The declaration on this form does have effect, though: it is the applicant's confirmation that what they have told you is true, and their authority for you to verify it.
- In most jurisdictions, yes β and that is what the declaration paragraph is for. It authorises you to verify what has been declared with the applicant's employer, bank and any credit reference agency, and to keep a copy of the form and its attachments. Have a lawyer in your jurisdiction check the wording against your own privacy notice before you print a run of them.
- The group version identifies the group rather than one person: its name, registration, date formed, and its chairperson, secretary and treasurer. It carries a roster for every member to sign, and a joint-and-several liability undertaking β meaning the lender may recover the whole outstanding balance from any one member, not just a share of it.
- Yes. The Word download is a real document, not a locked image β change the wording, delete a section, add your letterhead and save it as your own template. The form is free to use and has no watermark. It is a starting point rather than legal advice, so have it reviewed before you rely on it.
